Abstract
We report on our center’s clinical experience which supports the view that the new TIF Pinnacle sheath offers superior performance and enhanced safety. The complication rate was essentially nil and the use of closure devices was obviated altogether. However, these conclusions should be tempered by the limited nature of the study, mainly, the absence of a control patient subset, the potential built-in bias related to case selection and observation of outcomes, and its small size.

Abstract
Percutaneous coronary intervention (PCI) of a chronic total occlusion (CTO) remains a challenge for the interventional cardiologist. The introduction of novel techniques has greatly enhanced procedural success. One technique includes the use of a support balloon to improve guiding catheter support during recanalization of the coronary artery. We present a case report of a male who had a 100% occlusion of his right coronary artery for the last 27 years that we successfully recanalized. We also propose changes to the current balloon design that would potentially red

Abstract
Spontaneous upper limb deep venous thrombosis (DVT) is associated with exercise and occurs in young healthy individuals. Underlying anatomical abnormalities are often found, causing compression of the subclavian vein. Prompt thrombolysis and consideration of thoracic outlet decompression differentiate it from lower limb DVT. A case illustrating the clinical features of the condition is presented here.

Upper-limb deep venous thrombosis (DVT), Paget-Schroetter syndrome, or effort thrombosis, is a problem that is commonly misdiagnosed and improperly treated. The authors present a straightforward case of upper-limb DVT that appropriately highlights the diagnosis and management of this problem, while raising an important management issue. As mentioned by the authors, effort thrombosis usually occurs in younger patients. It is usually ac
